Description
The purpose of the Case Manager is to assist men and women who come to The Salvation Army Adult Rehabilitation Program (ARP) and the Transitional Living Program (TLP) for assistance with their physical, emotional and spiritual needs through a structured program of rehabilitation, work therapy, and spiritual regeneration. The Case Manager works directly with the Program Director to provide supervision and support to the individuals and/or families, conduct and facilitate outreach, provide information and referral services, case management, classes and group facilitation and follow up services. ESSENTIAL DUTIES AND RESPONSIBILITIES
KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS
Pursuing education in Human Services (e.g. social work, psychology or chemical dependency). Ideally, certification in Chemical Dependency Studies, Certification of Alcohol and Drug Counseling with recognized State Certified Agency, or AA/BA in related field.
While performing the duties of this job, the employee is regularly required sit for up to 2 hours operating a keyboard. Must be able to operate successfully in a rapidly changing work environment with multiple priorities. Ability to stand or be physically active for up to 2 hours or more at a time: must be able to walk, run, reach and grasp with hands and arms; lift, bend and stoop. The employee must occasionally lift and or move up to 25 pounds. To perform this job successfully, the incumbent must be able to perform the essential duties with or without accommodation.
