Position Summary

Continuum of Care (CoC) is a project with UNM's Department of Pediatrics, and has been serving people with Intellectual/Developmental Disabilities (I/DD) for over 30 years. CoC's mission is to improve and sustain the quality of health and healthcare for all people with I/DD in New Mexico.

CoC is proud to have a dynamic team of healthcare professionals who are committed to carrying out its mission. Now, we are seeking a skillful registered nurse to join the CoC team to fill the position of RN Case Manager. CoC takes great care of their staff by offering continual mentoring, training and supports. The RN Case manager will be in good hands.

Although the RN Case Manager is part of a clinical team, the ideal candidate must also have the proficiency in working independently to coordinate a variety of established specialty clinics. This position is not solo as they will work in tandem with the other RN Case Manager, so that together, all nursing duties will be covered. Job duties will include but not limited to:



  • Coordination of Specialty Clinics (i.e. CP Clinic, Neurology Clinic, etc.)
  • Assesses, provides, plans and evaluates care for patients in a wide variety of settings.
  • Triages phone calls and acts as liaison between physician, families, and patients.
  • Assists in problem solving with nursing staff from various community programs working with persons with intellectual/developmental disabilities on complex issues.
  • Arranges for specialized diagnostic testing such as EEG, Video EEG, MEG, MRI, etc.
  • Education of interdisciplinary members including nurses, physicians, families and patients regarding healthcare issues. Works in conjunction with physician to educate patients, families, schools and nurses on the CP, CP Ortho and Neurology clinics.
  • Manages and coordinates insurance issues related to the specialty clinics
  • Cross-trains with nursing staff to cover all nursing responsibilities in Continuum of Care
  • Design and present trainings on various developmental disability issues

Conditions of Employment
  • Possession of a valid New Mexico driver's license is a requirement for this job.
  • CPR Certified.
  • May be required to submit to annual TB and rubella screening.
  • UNM will provide Hepatitis vaccination, candidate may decline vaccination through declination form or may provide record of vaccination from previous employer.
  • Successful candidate must submit to post-offer, pre-employment physical examination and medical history check.


Minimum Qualifications

Associate's degree in Nursing; at least 1 year of experience directly related to the duties and responsibilities specified.
Certification/Licensure
State of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the multistate privilege to practice compact with New Mexico.

Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.



Preferred Qualifications

  • Experience in serving people with I/DD or related chronic conditions
  • Knowledge of New Mexico DD Waivers and I/DD systems
  • Knowledge of anticonvulsant medications
  • Ability to assist in problem solving for complex conditions such as Vagal Nerve Stimulator (VNS) used for treatment of seizures
  • Flexible and eager to learn the intricacies with treating patients with I/DD
  • Competence in working independently as well as an integral clinical team member
  • Knowledge of CP, CP Ortho, Neurology and Ketogenic Diet
  • Knowledge of infection control
  • Experience in case management, hospital or an outpatient clinical setting
  • Exemplary interpersonal communication skills - written and oral
  • Experience in designing and presenting trainings on I/DD topics
  • Exhibits solid communications and organization skills
  • Steadfast in continual learning to develop expertise in RN Case Management
