The Diabetes Educator possesses comprehensive knowledge of and experience in prediabetes, diabetes prevention, and management. The CDE educates and supports people affected by diabetes to understand and manage the condition. The CDE promotes self-management to achieve individualized behavioral and treatment goals that optimize health outcomes for the patient. The Diabetes Educator utilizes diabetes self-management education (DSME) strategies to develop an individual assessment and education plan developed collaboratively with the individual. The plan includes:

*Educational interventions directed toward helping the individual achieve self-management goals.

*Periodic evaluations to determine attainment of educational objectives or need for additional interventions and future reassessments.

*A personalized follow-up plan developed collaboratively by the individual and educator(s) for ongoing self-management support.

*Documentation in the education record of the assessment and education plan and the intervention and outcomes

Proactively makes educationally focused outbound calls to patients at risk for therapy attrition with the overall goal of recapturing lost patients or directing them to an educational event.

Provides ongoing patient educational support.

Facilitates webinars, conference calls, or other computer based training offerings.

Sets up and facilitates in-person and group education including diabetes education classes and pump starts.

Delivers exceptional customer service by embracing Grady values and standards of behavior during all patient, visitor and staff encounters.

Collaborates with the patient, family/caregiver, and health care team to develop and implement and individualized care management plan.

Conducts comprehensive patient assessments/reassessments.

Education:

Required: Registered Dietitian (RD) or Nutritionist, Masters Prepared Social worker or Masters Certified Health Education Specialist.

Experience:

Required: Two (2) or more years professional practice experience in an ambulatory setting or equivalent combination of education and experience in a hospital inpatient, outpatient, behavioral health or social service environment.

-Experience in motivational interviewing /health coaching / goal setting.

Required: Certification as Diabetes Educator (CDE) by the National Certification Board for Diabetes Educators (NCBDE) or willingness to complete within 1 year.

-Applicable license and certification must be current and active for the state of Georgia.

- Must have current American Heart Association BLS

-Excellent verbal and written communication skills are required.
