Job Summary:

The Health Equity Coordinator will work as part of a collaborative team to enhance and develop new partnerships, build trust, and align the Department of Family Medicine and Community Health's (DFMCH) Office of Community Health (OCH) efforts with university, clinical, and community groups. This individual will also collaborate to establish strategies to share power and center voices most impacted by inequities. The Coordinator will provide project management for initiatives co-created through community-clinical-university partnerships. Such initiatives include (1) Partnering in the creation of an LGBTQIA2S Project ECHO, a continuing education series, to increase clinicians' capacity to provide LGBTQIA2S Inclusive Care (2) Partner with DFMCH faculty to advance a Family Medicine focused assessment of clinicians and residents to guide quantity and content of future training and structural support focused on LGBTQIA2S health (3) Develop strategies to expand statewide partnerships and resource sharing in LGBTQIA2S health (4) Provide project management support to a team of clinicians, clinic managers, DFMCH faculty and staff in the development of Patient and Family Advisory Councils in four residency clinics (5) Partner with the DFMCH Grant Writer and OCH Team to seek and obtain grant funding focused on LGBTQIA2S health. (6) Support LGBTQIA2S focused Residents and Fellows in advancing their community-engaged learning objectives such as advancing Community Health Learning Experiences. The Health Equity Coordinator will work in partnership with the Office of Community Health team, Family Medicine Residency Community and Population Health Rotation Director, and the DFMCH Diversity, Equity, and Inclusion (DEI) Committee.


Responsibilities:
Delivers and assists with planning outreach program activities, strengthens partnerships, ensures adherence to policy, and assists with program administration to promote stable and productive relationships between the university and local community. Strives to expand and ensure access to programs, facilities, and educational services to diverse audiences. May assist with tracking and managing the outreach program budget.

  • 20% Identifies, promotes, and maintains external partnerships to support the outreach program
  • 10% Schedules and secures resources and communicates logistics in support of an outreach program
  • 10% Delivers outreach program content and materials to community members
  • 10% Researches, develops, and facilitates outreach program content and materials
  • 5% Develops and evaluates curriculum and programs and provides recommendations for improvement
  • 5% May assist with the monitoring of outreach program budget spending
  • 20% Leads the implementation of programs, policies, and procedures through oversight of day-to-day activities for program staff and student and community volunteers
  • 10% Assists with writing and submitting grant applications and proposals to secure research funding
  • 10% Conducts literature reviews, prepares reports and materials, and disseminates information to appropriate entities and assists with publications

Master's Degree in Public Health, social work, public affairs, or related discipline


Qualifications:

Required:
- Knowledge and understanding of social determinants of health, specifically LGBTQIA2S populations.
- Experience in public health, community-based healthcare organizations, and/or community-based healthcare programs.
- Demonstrated ability to work effectively with one or more of the following: community leaders, University faculty, physicians and with diverse community partners and local and state public health professionals.
- Excellent written and verbal communication skills, including group and process facilitation.
- High level of organization and attention to detail.
- Ability to work independently and as part of a collaborative team
- Ability to prioritize and manage multiple projects and initiatives.
- Proficiency in using word-processing, databases, and spreadsheets to complete tasks and projects, including Microsoft Office software.

Preferred:
- Specialization in partnership and coalition building, policy, systems and environmental change levers, and other strategies to advance community health.
- Demonstrated experience building community capacity to advance health and health equity at the state or local level.
- Experience with writing and submitting grant applications.
- Ability to manage grant activity and monitor outreach program budgets and invoices.
- Bilingual and/or proficiency in Spanish, Hmong, or ASL


Work Type:

Full or Part Time: 80% - 100%

It is anticipated this position requires work be performed in-person, onsite, at a designated campus work location.

The DFMCH Office of Community Health (OCH) aims to braid together the wisdom of our communities, our university, and our health systems to develop a sustainable infrastructure that will support the work of socially accountable healthcare initiatives focusing on advancing health equity.

We partner with community organizations to develop and implement evidence-based initiatives that promote community health and health equity, and that are likely to result in measurable improvements in population health outcomes. Through these partnerships, we are able to recruit and support faculty and staff to expand community-responsive health efforts; engage with patients and learners to initiative innovative models of care that promote community health; and evaluate these efforts to understand their impact.

University sponsorship is not available for this position, including transfers of sponsorship.
