Summary
The Registered Nurse is a professional nurse registered in Massachusetts who is responsible and accountable for planning and providing patient care for assigned patients in accordance with Nursing Standards of Care as set forth in the Nursing Department's Clinical Practice Manual. The registered nurse demonstrates initiative, knowledge and clinical skills in caring for the patient with complex needs. The Registered Nurse demonstrates the ability to effectively manage patients by assuming full responsibility for the assessment, plan, implementation and evaluation of patient care and is directly responsible to a designated nurse manager, or supervisor. ADDITIONAL DUTIES AND RESPONSIBILITIES: *Creates an authentic healing environment that supports the well-being of patients and their families. Supports the Department's Call Center and Clinical Triage Team to support triage efforts of urgent patient needs. *Ensures a seamless patient flow and positive patient experience across the care continuum in coordination of the individual practices throughout the department and its satellite locations. Works directly with the prescribers, clinical, and administrative teams as well as external agencies such as pharmacies, insurances and visiting nurse agencies to resolve patient issues and concerns. *Prepares, performs, and documents the coordination of clinical care, including assisting the patient with insurance and clinical components of paperwork, assisting with prescription refills, and other services to support the prescribers and other team members. *Collaborates with other team members on prior authorizations for prescriptions. Participates in the medication reconciliation process including medication review. *Provides patient education as needed and requested. Develops and maintains patient education materials in coordination with the department's clinical team. *Follow up telephone calls and other electronic communication regarding clinical questions as requested by providers *Participates in patient-centered health-maintenance efforts, including coaching patients towards health/wellness, disseminating information on condition, discharge medications, and follow-up care, which may include leading or participating in educational groups and hosting health-based educational series. *Collaborates with attending physicians, nurse practitioners, social workers, psychologists, community health workers, practice assistants and other team members as appropriate. *Assists the attending physicians and other prescribers in any potential collection and analysis of validated quality and outcome measure data. Collaborates on projects that impact overall departmental goals and initiatives. *May also participate in related research projects after completion of the required training. *Operations Support/Administrative Responsibilities: *Remains proficient with BWH computer systems as needed to independently perform daily tasks such as order entry, EMAR, lab tests and results, incident reporting, etc. *Consistently provides high quality and timely documentation, including admission notes, progress notes, telephone encounters, discharge summaries and dictations. *Participates in departmental education and training to medical assistants, other staff members and students regarding the needs of the department.
