Summary

We are hiring for a State RN Clinical Manager for Tennessee!

Clinical management experience is required.

This position will require travel across the state, as needed, to our various agencies.

The Clinical Director is responsible for providing direction to the patient care operations of the agencies providing Home and Community Based Services (HCBS) in the State. This includes: coordinating with and supporting the Clinical Managers in the HCBS locations to ensure compliance with Federal, State and MCO regulations, completing assigned projects to effectively support the immediate and long range objectives of the company; provides support of the eligibility of patients referred to home care services, planning for the services to be provided to patients and supervising their total home health care; implementing and maintaining administrative practices, agency philosophy, goals, and policies; enhancing the profitability of the agency; and providing motivation and retention of a qualified staff and assure the quality of services delivered. This position also acts as a liaison with management staff and other departments throughout the company.

Responsibilities

  • Directs and supports the Clinical Managers with oversight of patient care services and personnel within the state to include coordinating referrals and patient care, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the patient's individualized plan of care.


  • Directs and leads the Clinical Managers with providing staff conferences as needed and mandatory in-services, including but not limited to monthly staff/safety meeting.


  • Directs and supports the Clinical Managers with implementing and supervising patient care activities to ensure the delivery of safe, economical, and efficient patient care, which adheres to relevant standards of accepted nursing and medical practice, to include federal and state regulations, and Joint Commission standards, as well as to ensure compliance with payer source criteria.


  • Provides and oversees orientation for clinical personnel. Provides timely evaluation of clinical personnel.


  • Assures regularly scheduled continuing education for personnel and supervises compliance of staff with online learning management courses.


  • Acts as a liaison between staff, patient, community, leadership, and home office departments to ensure information is appropriately shared and communicated.


  • Assists Clinical Managers, on an on-going basis, ensures maintenance of records to support the qualifications, certification/licensure and competency of all clinical personnel.


  • Oversees weekly reports and key clinical metrics.


  • Directs and assists with timely and accurate billing, payroll, and processing of clinical workflow.


  • Participates in the Emergency Preparedness plan and implementation during emergencies to ensure appropriate plan execution, as directed.


  • Participates in the clinical on-call rotation as needed.


  • Participates in the organization's Performance Improvement and Compliance program and adheres to policies and procedures.


  • All other duties as assigned.



Education and Experience

Job Qualifications:

Current RN licensure in state of practice.

Current CPR certification is required.

Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation.

Additional State License Requirements:

For Category I and II Medicare Certified Home Health:

KY, NY, OH, TN, WI: : No additional requirements.

For Category III and IV:

AR, CT, PA, GA, MO, NC, NY, PA, TN, VA: No additional requirements.
