Care Continuum Manager, Care Management

The Care Continuum Manager ensures that patient transitions through the care continuum are patient-centered and streamlined for both internal and external key stakeholders. This includes, but is not limited to, promoting seamless care across the continuum, coordinating communications and activities between Norton Healthcare and post-acute partners, utilizing information provided via monthly reports to establish appropriate actions and drive performance excellence across the continuum and assessing models to evaluate business plans for potential acquisitions or new ventures. The Care Continuum Mgr serves as a key resource within Norton Healthcare's Improvement Network promoting optimal value-based care for patients through strong collaboration with leaders across the system.

Key Accountabilities:

• Understand referral/admit status of each Norton Healthcare hospital, as well as post-acute facilities, bed availability to facilitate appropriate referrals and admissions. Assist with engaging and setting expectations of medical staff and NHC hospital leadership to understand reason/perceptions of care provided at various post-acute facilities to support timely and appropriate transition of patients from the acute care setting.
• Oversee the daily operations of care coordination. This includes, but is not limited to, ensuring the effective utilization of care coordination staff, providing guidance to the care coordination staff in patient care management related issues, as well as mentoring, coaching and evaluating performance of staff.
• Collaborate with internal NHC departments along with post-acute facilities to drive standardized education content and materials for implementation across the post-acute continuum.
• Assess models and evaluate business plans for potential acquisitions and new ventures.
• Support strategic partnerships between Norton Healthcare and post-acute partners across the entire care continuum to drive top quality service. Receive, review and interpret various reports related to partnerships to identify opportunities to improve performance based on local or national standards and drive action items, as well as Performance Excellence collaboration.
• Collaborate with community partners, hospital care management, ambulatory nurse navigators, managed care/payors and other key stakeholders to maintain and improve quality of care and patient satisfaction.

Required:

• Three years in a healthcare or business environment
• Bachelor Degree

Desired:

• Master Degree
