The Clinical Coach is an experienced and competent RN who assists the newly licensed RN (Nurse Resident) to gain competency, confidence and comfort in meeting the needs of hospice patients and families.
Primary tasks, duties, and responsibilities:
1. Promotes and practices the Hospice mission and values and follows policies and procedures of the organization.
2. Demonstrates knowledge of and has ability to teach adherence to regulatory requirements related to nursing practice and the provision of hospice care (e.g., Nurse Practice Act; Centers for Medicare and Medicaid Services (CMS); Agency for Healthcare Administration (AHCA); and Community Health Accreditation Partner (CHAP).
3. Demonstrates and follows the American Nurses Credentialing Center Transition to Practice curriculum.
4. Demonstrates willingness to teach, coach, role model, and evaluate the Nurse Resident.
5. Utilizes clinical expertise to foster the educational development and competency of the Nurse Resident.
6. Designs learning plans to meet the educational needs of the Nurse Resident.
7. Collaborates with the management team to develop an individualized learning plan for the Nurse Resident.
8. Implements learning activities using adult learning principles to foster development of the Nurse Resident.
9. Evaluates effectiveness of learning activities in achieving educational goals.
10. Communicates progress of Nurse Resident to his/her manager/supervisor at pre-determined, regular intervals.
11. Supports nurses in the Nurse Residency Program by providing coaching and bedside teaching.
12. Provides timely developmental feedback to the Nurse Resident regarding performance.
13. Ensures Nurse Resident completes all required modules assigned via the learning management system (LMS).
14. Maintains confidentiality of the evaluation and progress of the Nurse Resident.
15. Collaborates with Hospice Education Specialists as needed to identify/create learning opportunities for Nurse
Resident.
16. Provides guidance for the proper utilization of computers in patient care and appropriate electronic medical record
(EMR) documentation.
17. Serves on organizational committees as appropriate.
18. Participates in education activities that support the clinical coach's professional development.
Educational/Professional:
1. Graduate of approved nursing program with current, unrestricted licensure as a RN in Florida, BSN preferred.
2. Minimum 1 year experience as an Empath Hospice RN.
3. Experience as a Certified Empath Preceptor and/or educator preferred.
4. CHPN certification strongly preferred; or obtained within two years.
