Nurse Navigator

The Cancer Center of Kansas (CCK) is seeking a full-time, on-site Nurse Navigator to join our team in our Wichita, KS offices. The typical work week is Monday through Friday, 8:30 AM - 5:00 PM with no major holidays, no on-call and no weekends.

What does this position entail?

Under general supervision, the Nurse Navigator provides individualized support to patients, families, and caregivers in a community oncology setting. Using the nursing process, the role involves educating patients, facilitating access to internal and external resources, and ensuring informed decision-making and timely symptom management. The position requires frequent collaboration with internal departments (e.g., scheduling, pharmacy, nursing, lab, imaging) and external referral resources. The Nurse Navigator works closely with physicians, practice leadership, and clinical staff to promote and guide the patient navigation process. Supports and adheres to the US Oncology Compliance Program, including the Code of Ethics and Business Standards.

The essential duties and responsibilities of this position are:

• Assesses health status by interviewing patients about their medical history and current status, documenting relevant nursing assessments and interventions.

• Reviews and reports health status assessments, including lab results, diagnostic imaging, and other relevant findings, taking appropriate action based on standard or verbal orders.

• Develops nursing diagnoses and corresponding action plans as necessary.

• Provides patient education regarding therapy, disease stages, preventive care, symptom management, and medications.

• Identifies patient/family educational needs and provides appropriate teaching materials and instructions.

• Offers psychosocial support to patients and their families.

• Manages medication refill requests from pharmacies, documenting details in patient charts.

• Advocates for patients and supports their empowerment in the care process.

• Facilitates referrals as part of the care plan, treatment course, or advance care planning, in collaboration with scheduling teams, and as directed by a provider.

• Coordinates consultations, diagnostic tests, procedures, and follow-up appointments for patients.

• Triages patient inquiries, including phone calls and messages, documenting key information in patient charts, and coordinating communication between patients, families, and the healthcare team.

• Acts as the patient liaison between healthcare providers and patients.

• Assists patients in accessing community resources as needed.

• Engages in professional development activities to stay current in oncology nursing practices through ongoing education.

• Participates in community outreach initiatives, focusing on cancer prevention and education.

• Plans, implements, coordinates, and evaluates continuity of care, ensuring adherence to established standards and practices.

• Maintains compliance with all relevant laws and regulations, including confidentiality, HIPAA, and state and federal guidelines regarding patient records and information.

• Keeps licensure current and meets all continuing education requirements.

• Demonstrates knowledge of nursing practice guidelines, healthcare economics, and local healthcare community dynamics.

• Manages e-prescribing under physician orders.

• Completes referral and certification paperwork for established patients as required.

• Maintains the patient health record with details regarding coordination and management of care facilitated.

• Demonstrates strong organizational and critical thinking skills, including prioritizing and re-prioritizing quickly.

• Maintains patient confidentiality in accordance with legal and organizational standards..

• Follows all applicable policies and procedures set by the company and Network.

• Attends mandatory meetings and participates in required organizational activities.

• Assumes additional duties as assigned, including nursing coverage.

• Performs other tasks as assigned.

The ideal candidate for the position will have the following qualifications:

Education, Certification and Experience:

• Graduate from an accredited program for professional nursing education required.

• RN state license and current registration with the State Board of Nursing in practicing state.

• BSN preferred.

• Must maintain continuing education requirements.

• 3 years nursing experience, preferred.

• Current CPR or BLS certification required.

• Current Kansas nursing license required.
