Methodist Hospital is looking to hire a Manager, Hospital Care Management. Work Schedule: 1.0 FTE - 8:00am - 4:30pm with occasional weekend/evening leader on call POSITION SUMMARY STATEMENT: To provide supervision and leadership for the Hospital Care Management team, including RN Care Managers, Social Work Care Managers, RN and Social Work Supervisors/Leads, support, and other affiliated staff in both the inpatient and Emergency Center settings. The manager will assure the successful development and implementation of care coordination strategies as they relate to the department. To act as a clinical and process consultant for the team and hospital regarding care coordination and transition/discharge planning. To actively support hospital care and system improvement initiatives with increased focus on identified areas of opportunity with hospital care management and affiliated activities. MINIMUM QUALIFICATIONS:
- Education, Experience or Equivalent Combination:
- Master's degree in Social Work and/OR Bachelor's/Master's Degree in Nursing
- Minimum of 3 years clinical practice experience in health care setting, with specific experience in care management, care coordination or discharge planning.
- Minimum 3 years leadership/management experience
- Licensure/ Registration/ Certification:
- Licensed Independent Clinical Social Worker (LICSW) licensure and/OR Registered Nurse (RN)
- Current license from the Minnesota Board of Social Work and/OR Minnesota Board of Nursing
- Knowledge, Skills, and Abilities:
- Pager, vocera, computer, electronic medical record, fax, scanner
PREFERRED QUALIFICATIONS:
- Education, Experience or Equivalent Combination:
- Master's Degree in Social Work or Nursing
- Licensure/ Registration/ Certification:
- Clinical Supervisor Certification through the Board of Social Work if LICSW
- Knowledge, Skills, and Abilities:
- Demonstrated effective leadership and supervisory skills.
- Demonstrated experience in hiring, orientation, coaching, and team building of professional staff.
- Demonstrated flexibility and organization.
- Demonstrated appropriate decision making under challenging situations.
- High level of expertise in written, oral, and interpersonal communication.
- Demonstrated skill in effective use and management of automated medical management systems.
- Understanding of healthcare and/or managed care industry.
