Position Summary: Job Responsibilities: Performs other duties as assigned. Collaborates with private physicians to facilitate communication between the hospital, physicians, and patients regarding status updates and follow-up needs. Participate in daily discharge rounds to facilitate patients being discharged before noon. Performs the required trauma screenings including documentation in screening tools, such as: RAPP, SBIRT, ITSS, PTSD, etc. Drug and alcohol screening and appropriate interventions, including referral to community resources if indicated. Acts as a Trauma Survivors Network Coordinator, and runs appropriate programs for patients and community members. Oversees compliance ACS and DOH standards and regulations. Perform follow-up calls to patients and families to ensure continuity of care. Collaborate with care management and nursing teams to facilitate patient discharges. Trouble shoots for patients, assists with emergency appointments. Recommends policies and programs that will help improve and ensures the quality of care rendered by the Division. Contributes to growth and development of self, co-workers through formal and informal education. Assists and supports team members. Keeps current with knowledge in area of practice and related issues. Attends and participates in staff/practice meetings, and in-services. Ensure clear communication with all relevant parties, including physicians, surgeons, nursing staff, and care management teams. Participate in service line staff meetings to address patient care and operational needs. Collaborate with service line members to streamline transitions for surgical patients. Obtains necessary medical, nursing and other staff insights from appropriate personnel and/or medical records to assist in meeting and determining patients needs. Assist with scheduling necessary outpatient ACS clinic follow-up as well as sub-specialty follow-up if indicated. Attends trauma specific meetings as needed to resolve problems or share information. Communicates with other team members exchanging information re: social, emotional and environmental problems, which may affect patients response to treatment. Partner with nursing and patient relations teams to enhance the patient experience. Utilize appropriate tools and metrics to monitor and assess patient satisfaction levels. Develop and implement initiatives to address feedback and improve outcomes. Serve as a liaison for selected outreach initiatives and partnerships. Direct patients and their significant others to appropriate community resources and services, long term family counseling and so forth. Perform post discharge phone calls to reduce return to the emergency room visits and potential readmissions. Assist with injury prevention and outreach initiatives, such as: Stop The bleed, falls prevention and teddy bear clinics. Maintain data for quality initiatives across service lines. Extract and analyze data for quality core measures and dashboards. Organize and document specialty service line meetings at both regional and annual levels. Assist in developing quality improvement initiatives and dashboards for service lines. Participates in the Performance Improvement Committee Assists In orienting and training new personnel and serves as a resource for other personnel in resolving patient care problems. Monthly staff meetings. Participates in departmental committees. Exhibits a working knowledge of the departments performance improvement functions by understanding and gathering data for studies and such. Complies with Department of Health regulations regarding care to patients and documentation of it. Accepts assignments based on demonstrated competence to provide care to age and gender specific population. NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $112,360.00 - $151,431.17 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
