In close partnership with patients, families, physicians, case managers, and other healthcare professionals, this role provides direct social work services including comprehensive assessments, behavioral interventions, goal setting, and care plan development. You'll be at the forefront of care coordination-breaking down psychosocial barriers, crafting discharge strategies, connecting patients to critical resources, and guiding them through complex transitions. From crisis intervention to patient advocacy, you'll be a key voice in optimizing outcomes and ensuring every patient receives holistic, high-quality support. Key Responsibilities:

Salary Range: Minimum $31.92/hour - Maximum $52.69/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
