Description

The LTSS Program Coordinator provides administrative support for Long-Term Services and Support (LTSS) waiver programs and LTSS applications. The Coordinator will collaborate with staff within the organization and external partners and providers. The LTSS Programs Coordinator will work independently and as part of teams, compiling and reviewing reports of LTSS while providing excellent customer services skills with external stakeholders.

Duties and Responsibilities

Responsibilities include, but are not limited to the following:



  • Assist with compiling reports for LTSS applications from internal and external providers
  • Collaborate with home care agencies as appropriate
  • Assist in reviewing and validating data from LTSS reports
  • Remain current with EOHHS changes to the LTSS application and change forms
  • Collaborate with clinical teams on management of waiver submissions for non-Custodial members in custodial care and assist with follow-up to facilities for LTSS application process and status.
  • Assist with monitoring pending LTSS Application list and report findings to Director monthly
  • Provide assistance with new and ongoing Pilot programs and working with relevant business units
  • Maintain calendar and reporting for LTSS applications for determination
  • Maintain Home Care agencies and CAP quarterly and ah-hoc meetings and minutes.
  • Maintain knowledge of the business and regulatory environment as it relates to LTSS
  • Participate in meetings as appropriate
  • Participate in Outreach/Marketing activities as needed and appropriate
  • Perform other duties as assigned
  • Corporate Compliance Responsibility - As an essential function, responsible for complying with Neighborhood's Corporate Compliance Program, Standards of Business Conduct, applicable contracts, laws, rules and regulations, policies and procedures as it applies to individual job duties, the department, and the Company. This position must exercise due diligence to prevent, detect and report unlawful and/or unethical conduct by fellow co-workers, professional affiliates and/or agents


Qualifications

Qualifications

Required:



  • Associate Degree or equivalent years of background and experience to equal degree
  • Minimum of one (1) to three (3) years of experience working in a health payor, provider or social services environment
  • Strong oral and written communication skills, including excellent customer service and interpersonal skills
  • Proven ability in prioritizing and completing multiple task assignments and meeting deadlines in a flexible environment
  • Strong organizational and prioritization skills
  • Strong administrative and clerical skills
  • Knowledge in medical terminology
  • Ability to work independently with minimal direction and collaboratively as part of a team
  • Ability to keep accurate records and prepare reports
  • Critical listening and thinking, time management, decision making/problem solving skills
  • Ability to build and maintain relationships internally and externally
  • Intermediate to Advanced skills in Microsoft Office (Excel, Outlook, PowerPoint, Word)


Preferred:



  • Bachelor'sDegree
  • Knowledge and experience with LTSS/HCBS services and service delivery coordination
  • Experience with CAP/Home Care agencies
  • One (1) to Three (3) years in an HMO/MCO environment
