Position Summary:

The Case Aide will assist Care Coordinators with maintaining and distributing member rosters, enrollment and discharge reports. Case Aide will be in a supportive role to the Care Coordinators, Program Directors, and Statewide Director. The Case Aide will perform a variety of tasks related to the management of a caseload: entering data into computer systems, outreach to members, and assisting with documentation and timeline requirements set forth by Lutheran Social Service of Minnesota (LSSMN), the contracted Managed Care Organization's (MCO's), Minnesota Department of Human Services (MNDHS), and Centers for Medicare and Medicaid Services (CMS).

Position Requirements:



  • High School diploma required; Associate degree is preferred
  • Prefer 1 year of post-secondary education OR administrative experience, office/clerical experience, case aid or closely related experience
  • Experience in the Human Services Field preferred, but not required
  • Valid driver's license.
  • Must pass background study and motor vehicle check.
  • Strong written and verbal communication skills.
  • Computer skills to include basic knowledge of software such as Microsoft Office Suite.
  • Detailed-oriented, highly organized, and possess analytical and problem-solving skills.
  • Able to work independently and to participate actively in a team.
  • Able to implement strategies to enhance delivery of services to culturally diverse populations.
  • Able to recognize boundaries and exhibit patience, self-control and ability to adapt to a dynamic environment.
