*Professional Summary:* The Home Health Plan of Care (POC) Specialist is responsible for ensuring the timely creation and accuracy of Plan of Care (POC) documents in compliance with CMS guidelines and billing requirements. This role ensures regulatory compliance, conducts audits, provides education to clinical staff, and collaborates with leadership to enhance POC accuracy and efficiency. The ideal candidate is detail-oriented, proactive, and committed to maintaining high standards in home health documentation and regulatory adherence.
*Key Responsibilities:* * Prepare and finalize Home Health Plan of Care (485) documents to meet CMS guidelines and billing standards. * Ensure regulatory compliance by verifying the accuracy and completeness of POC components. * Conduct daily reviews of assigned POC tasks to maintain quality and efficiency. * Assist with quarterly audits and other team-related compliance functions. * Work closely with leadership to assess performance trends and determine educational needs for clinical staff. * Provide training and constructive feedback to clinical staff on proper completion of POC documents. * Identify problem areas proactively and report concerns to management. * Perform additional responsibilities as assigned to support compliance and operational effectiveness. *Qualifications:* * Strong knowledge of home health and CMS regulations for Plan of Care, including both clinical and billing aspects. * Demonstrated ability to coach, mentor, and educate staff effectively. * Excellent analytical skills and decision-making abilities. * Proficient in written, verbal, and interpersonal communication. * Ability to understand clinical policies and clearly articulate health record requirements. * Capable of prioritizing workload to meet tight deadlines in a fast-paced environment. * Proactive in recognizing potential challenges and implementing solutions. * Intermediate proficiency in Microsoft Outlook, Excel, Word, and PowerPoint, with a willingness to learn new technology. * Committed to delivering high-quality work and ensuring compliance with regulatory standards. *Pay and Benefits* The pay range for this position is $17.26 - $25.87/hr.
