Responsibilities

Join Our Team as an AID Patient Intake Coordinator!

Your Role: As the AID Patient Intake Coordinator, you will be the first point of contact for patients seeking our services. Your primary responsibility will be to handle both external and internal requests for information and services across all levels of care. You will play a vital role in assessing needs, scheduling appointments, and providing crucial support during times of crisis.

Key Responsibilities:

  • Connect and Support: Review and assess incoming calls, linking patients with the appropriate services and resources they need.
  • Crisis Management: Triage crisis phone calls, assess safety, and provide county crisis resources, ensuring that individuals receive timely and appropriate support.
  • Mental Health Screening: Conduct Mental Health Screens over the phone, gathering essential information to facilitate care.
  • Appointment Scheduling: Schedule Admission Diagnostic Assessments and intake appointments for both program and outpatient clinic services, ensuring a smooth transition into care.
  • Referral Coordination: Facilitate both internal and external referrals, consulting with physicians as necessary to streamline the process.
  • Customer Relations: Provide outstanding customer service in line with PrairieCare's policies, ensuring that every interaction is positive and supportive.
  • Online Engagement: Monitor and respond to patient inquiries through our website, following up as needed to ensure all questions are addressed.

Qualifications

  • Education: A Bachelor's degree in Social Work, Sociology, Psychology, or a related field is required to help you understand complex mental health issues.

  • Experience: At least two years of supervised experience in behavioral healthcare is needed, ideally qualifying you as a mental health practitioner. A Licensed Social Worker (LSW) license is a big plus!

  • Crisis Management: Experience in handling psychiatric emergencies is preferred, as your ability to assess and respond to urgent situations is crucial.

  • Clinical Knowledge: A strong understanding of DSM-5 criteria and solid interviewing and assessment skills are essential for professional interactions.

  • Cultural Competence: Familiarity with gender and cultural differences, as well as age-appropriate care, will enhance our patient services.

  • Communication Skills: Exceptional verbal and written communication skills are necessary for clear interactions with clinicians and patients.

  • Tech Skills: Proficiency in Microsoft Office, especially Word and Outlook, is required to streamline processes and communication.

Compensation for this role ranges from $22-28 per hour depending on education and experience.
