Community Intervention Specialist - Homeless Outreach

Department of Health & Human Services

Hourly Range: $28.22 - $32.85

The mission of the Milwaukee County Department of Health & Human Services is to enhance the quality of life for individuals who need support living healthy, independent, and safe lives within our community. Job Summary: This position is focused on providing full-time street outreach services to individuals and families experiencing homelessness in Milwaukee County. This position will work to secure indoor placements for consumers experiencing homelessness. Services will include street outreach services, case management, linkage to mental health/substance abuse treatment, and assisting consumers in finding and maintaining permanent housing.

Job Duties:



  • Street outreach services. Locate and engage individuals and families experiencing unsheltered homelessness, provide housing-focused street outreach services, and collaborate with outreach teams to develop immediate housing plans and refer to permanent housing programs.
  • Coordinate care. Maintains linkage with outreach teams to find successful housing placements.
  • Benefits acquisition. Assist homeless individuals with gaining income and insurance.
  • Additional units. Work with existing and potential landlords to find additional units and to ensure a quality of housing for homeless placement.
  • Community Partners Collaboration. Communicate and collaborate with community partners regarding Milwaukee County Housing Services and to represent the Street Outreach team in meetings. This is to include partners such as funders, law enforcement, elected officials, citizens, and businesses.
  • Perform other duties as assigned.


Minimum Requirements:



  • Current resident of the State of Wisconsin.
  • Current Wisconsin Driver's License and access to a reliable insured vehicle; both maintained throughout employment.
  • Must possess a high school diploma and/or GED equivalent.
  • At least one (1) year of experience working with the homelessness population, supportive housing, and/or Human Services.
  • At least Intermediate knowledge of all computer and software applications such as Microsoft Word and Microsoft Excel.
  • Must be able to travel extensively, up to 70% of work time, within the community.


Knowledge, Skills, and Abilities: Knowledge of housing programs and HUD regulations as they relate to homelessness and income guidelines; knowledge of available housing resources; knowledge of the permanent supportive housing model; ability to conduct clinical assessments to provide for proper discharge planning; ability to work with individuals with a wide range of needs; ability to effectively communicate; ability to read, write, instructions, reports, short correspondence and memos.
