Job Summary

We are seeking a motivated and compassionate Community Health Worker (CHW) Intern to join our aiECHO Healthy Baby Connect team. This position is specifically geared toward supporting the perinatal and postpartum population via home visits, clinic visits and/or telehealth visits. The CHW Intern will play a crucial role in supporting families, providing education, and facilitating access to healthcare services. A passion and/or experience for maternal, infant, and child health is preferred. This internship offers a unique opportunity to gain hands-on experience in community health and maternal-child health. About aiECHO Healthy Baby Connect: aiECHO Healthy Baby Connect is a comprehensive program designed to support the health and well-being of infants and their families. This program is administered via home visits, clinic visits and/or telehealth visits. The program leverages advanced technology to track and monitor various aspects of maternal care, surrounding Social Determinants and Behavioral Health needs. Our goal is to provide parents and caregivers with the tools and resources they need to ensure their babies thrive.

Essential Functions

Data Collection and Processing

  • Assist in conducting AI-powered social needs assessments to generate a Health-Harming Social Index (HHSI) for pregnant women.
  • Document and process data in Excel and/or EMR system, including patient information, identified needs, and priority scores.

Prioritization

  • Sort and prioritize cases based on the number of identified social needs.
  • Focus on women with the highest number of identified needs.

Outreach Preparation

  • Review specific needs for each individual to tailor outreach efforts.
  • Prepare outreach scripts, resources, and referrals relevant to common needs.

Outreach and Engagement

  • Initiate contact with prioritized participants, introduce the program, and obtain consent for participation.
  • Conduct comprehensive screenings via home visits, clinic visits and/or telehealth visits to evaluate social needs across various domains.

Documentation

  • Record interactions, communication outcomes, actions taken, and updates to identified needs in the Excel document.
  • Maintain detailed S.O.A.P. notes for each participant.

Follow-Up

  • Schedule and conduct follow-up calls/visits/emails to address unresolved or ongoing issues.
  • Monitor progress on referrals provided and document outcomes.

Reporting and Case Conferences

  • Generate weekly reports summarizing contacts, needs addressed, and outstanding issues.
  • Participate in weekly case conferences to discuss progress and challenges with the care team.

Continuous Improvement

  • Collaborate with the care team to refine workflows and improve resource mapping and referral processes.
  • Provide feedback on unaddressed needs or misidentified issues to enhance the AI tool and CHW workflows.
Qualifications

Required

  • High School Diploma or equivalent and successful completion of the paraprofessional CHW Training Program (Completion within 180 days from hire date)
  • Driver's License - State of Michigan - upon hire
  • Auto Insurance - upon hire

Preferred

  • Associate's degree or equivalent
  • 3 years of relevant experience
  • Knowledge and use of community resources; social, health and public agencies and services.
