Job Summary

The Division of Geriatrics seeks to recruit a full-time Age-Friendly Health System (AFHS) Quality Improvement (QI) Specialist to support and implement new and ongoing programs and initiatives. The Age-Friendly Health System program is an implementation and quality improvement team directed to improve care for older adults across the UCSF health system and community by improving complex interdisciplinary workflows, increasing interdisciplinary skills and knowledge in caring for older adults, and standardizing and simplifying processes across the different segments of care. Reporting directly to the Age-Friendly Health System Medical and Nursing Directors, the AFHS QI Specialist will coordinate and implement programmatic, quality improvement, and coordination efforts on all AFHS clinical and QI programs specifically to improve costs and quality outcomes for all older patients at UCSF Health. The AFHS QI Specialist will also provide non-clinical support, guidance and assistance for patients and families. Tasks may include: daily coordination with the care team including supporting discharge, post-discharge and telehealth needs, addressing patient grievances and making recommendations, coordinating volunteer activities including implementation and oversight of the Acute Care for Elders (ACE) volunteer program, managing unit space, files and other resources (such as software/hardware), overseeing websites and outward-facing content, organizing AFHS or other Department events, making program-related purchases, submitting reimbursements, as well as supporting special projects aimed at optimizing care and care outcomes where needed. The AFHS QI Specialist is also expected to be able to identify unmet needs and opportunities for improved patient experience and share with leadership, oftentimes providing meaningful suggestions and recommendations for improvement. Their day-to-day functions may include: coordinating and leading meetings with stakeholders, project management including creating agendas/minutes and following up with various stakeholders, supporting budgetary functions, as well as extracting insights from data, including the analysis, collection, organization, reporting, display and storage of data to health system leaders and frontline stakeholders. The AFHS QI Specialist will work closely with the Medical Director and the leadership team to generate reports, evaluations and metrics for various stakeholders to ensure all program goals/deliverables are met and may be required to present the data at meetings. They are also expected to acquire new skills and gain sufficient proficiencies in various software as they become available/required.

Our primary goals are to sustainably increase patient satisfaction, improve and optimize clinical care and cost outcomes across the spectrum of care, and effectively communicate these outcomes to health system leaders and frontline stakeholders. A successful candidate will be able to implement/coordinate concurrent projects efficiently, track quality improvement activities, promote program awareness, serve as the "face of the AFHS" (as in the case of media coverage), advocate for and guide patients and their families to a broad range of services, amenities, and information to promote healing and ensure satisfaction with the patient care experience throughout the community and the UCSF Health system.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $78,900 - $167,300 (Annual Rate).

The Division of Geriatrics' work is grounded in a deep commitment to and respect for older persons. We are specialists in aging. Our expertise is in optimizing the health of older individuals. We work in the community and on virtually every UCSF campus - Parnassus, San Francisco VA Health Care System, Zuckerberg San Francisco General Hospital, Mission Bay, and UCSF Center for Geriatric Care. The Age-Friendly Health System includes all the inpatient and outpatient clinical Geriatrics programs, as well as multidisciplinary harm reduction programs throughout the hospital and system. The mission of the Age-Friendly Health System is to provide the best care for hospitalized and transitions for older adults with a focus on maintaining and improving physical and cognitive function and ensuring safe transitions from hospital or Geriatrics ED to other sites of care including skilled-nursing facilities and home.

Required Qualifications


  • Bachelor's degree in a healthcare-related field, or a combination of comparable education and/or related work experience.
  • Minimum of two (2) years of relevant work experience.
  • Working knowledge of QI standards, clinical chart review and abstraction, and regulatory requirements.
  • Working knowledge of QI models, tools, and techniques, including collection, aggregation, abstraction, analysis, and reporting of data.
  • Detail oriented, with demonstrated organizational skills and the ability to complete projects and tasks on a timely basis.
  • Demonstrated problem solving skills to assess problems quickly and develop multiple options for effective and timely resolution.
  • Demonstrated interpersonal skills for collaborating effectively with a range of clinical and technical staff and representatives of external regulatory agencies.
  • Demonstrated ability to effectively convey complex clinical and technical information both verbally and in writing, and to produce reports, presentations and documentation.
  • Working knowledge of various administrative and business software packages, and of the specialized applications for quality data management.
  • High proficiency with Windows-based software including Microsoft Word, Excel, PowerPoint, Outlook, and ability to acquire high proficiency in other job-related software.
  • Ability to work onsite in a clinical care setting and offsite/remotely as needed.
  • Ability to work in fast-paced environment, requiring little to no supervision. Ability to multi-tasks and meeting multiple competing deadlines. Strong organizational skills required.

Preferred Qualifications


  • American Heart Association Basic Life Support (BLS) Certification
  • Strong knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Knowledge of Medical Terminology. Strong knowledge of data collection, compilation, and analytical techniques. Proficiency with EPIC Apex or other healthcare applications.
  • Strong skills to comprehend and assess patients' grievances to quickly locate an appropriate resource for assistance. In-depth knowledge of the organization and how to get issues resolved.
  • Prior experience developing workflow procedures and implementing systems to improve operations. Strong project management skills.
  • Strong interest and/or experience working with older adults.
  • Bilingual/Multilingual.
