Responsibilities

Are you ready to make a profound impact in the lives of young individuals facing mental health challenges? At our 101-bed Psychiatric Hospital in Brooklyn Park, Minnesota, we are seeking a dedicated Inpatient Social Worker to join our dynamic team. In this vital role, you will carry a patient caseload, working with adolescent and young adult patients aged 5 to 35 who have an average length of stay of 7-10 days. Your expertise will be crucial in facilitating group therapy, crafting treatment and discharge plans, and coordinating care with a variety of stakeholders.

What You'll Do:

As an Inpatient Social Worker, you will be at the heart of our treatment efforts, collaborating daily with Psychiatrists and Therapists as part of the primary treatment team. Your responsibilities will include:

  • Clinical Expertise: Utilize your knowledge of emotional and behavioral health practices to assess, plan, implement, and evaluate psychosocial services for our patients.

  • Treatment and Discharge Planning: Develop and revise comprehensive treatment and discharge plans, ensuring clear communication with patients, caretakers, the treatment team, and direct care staff.

  • Psychoeducation: Provide valuable education to patients and their families about diagnoses, symptom management, and discharge plans. You'll lead discharge meetings and discuss safety plans, patient rights, and financial considerations.

  • Community Connections: Build strong relationships with external providers, agencies, and payer networks to ensure seamless transitions to the next appropriate levels of care after hospitalization.

  • Group Facilitation: Lead engaging therapy and process groups for children, adolescents, and young adults on the inpatient units, fostering a supportive environment for healing and growth.

  • Advocacy and Ethics: Commit to providing patient-centered, culturally appropriate, and age-appropriate care, advocating for the needs of each individual you serve.


Qualifications

  • Requires a Master's Degree in Social Work. Requires licensure in the State of Minnesota, LGSW required, LICSW preferred.
  • Two years' experience in a mental health care setting preferred.
  • Experience working with children, adolescents and their families strongly preferred.
  • Experience using electronic health records preferred.
  • Knowledge of Microsoft Office products including Word, Outlook, and Teams.
  • Ability to communicate in written and verbal form effectively and efficiently.
  • Ability to coordinate care with relevant staff.
  • Ability to shift between dissimilar tasks smoothly and rapidly.
  • Ability to make effective decisions while in stressful situations and circumstances.

Compensation for this role ranges from $26-34 per hour depending on education and experience.
