GENERAL SUMMARY: Responsible for the identification, abstraction and follow-up of clinical data from the medical record, patient, and treating physicians in order to maintain a complete and accurate cancer registry database under the supervision of an Oncology Data Specialist - Certified. Fulfills responsibilities related to American College of Surgeons (ACOS) accreditation including quality of cancer registry data and cancer conference coordination. PRINCIPAL JOB FUNCTIONS: 1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. 2. *Identifies all patients seen at the Medical Center with a diagnosis qualifying for inclusion in the Registry by reviewing pathology reports, cytology reports, bone marrow reports, autopsy reports, and disease indices. 3. *Under supervision of an Oncology Data Specialist - Certified, abstracts data, including patient demographics, diagnostic procedures, history and extent of disease, treatment, follow-up, physician, and related information from medical records; codes data into database according to state and national requirements. 4. *Reviews data for completeness and accuracy and follows-up with data sources as necessary. 5. *Codes cancer diagnosis for the Registry using ICD-O. 6. *Submits cancer registry data for edits; submits finalized cancer registry data to the state and National Cancer Data Base. 7. *Performs annual follow-up on applicable registry patients. 8. *Ensures compliance with American College of Surgeons standards for cancer registry data; collaborates with the Cancer Committee to establish and implement conference goals and objectives. 9. *Coordinates weekly Cancer Conferences for maximum physician participation. 10. *Coordinates the development and implementation of the cancer registry quality control plan; monitors and reports the results of the cancer registry quality control activities to the Cancer Committee. 11. *Monitors and reports Cancer Conference activities to the Cancer Committee, recommending corrective action as necessary. 12. *Compiles and submits data for special studies and projects required by the American College of Surgeons. 13. *Assists the Cancer Program Administrator with abstracting data for Patient Care Evaluation Studies, calculating statistics or other projects required by the American College of Surgeons. 14. *Assists the Cancer Program Administrator in the preparation of the annual report and in providing administrative support to the Cancer Committee. 15. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. 16. Participates in meetings, committees and department projects as assigned. 17. Performs other related projects and duties as assigned. (Essential Job functions are marked with an asterisk "*". Maintains clinical competency as required for the unit including but not limited to age-specific competencies relative to patient's growth and developmental needs, annual skill competency verification and mandatory education and competencies. 2. Knowledge of quality improvement and quality control methods and processes. 3. Knowledge of medical terminology, anatomy, physiology and disease processes. 4. Knowledge of the American College Of Surgeons requirements for an approved Cancer Registry. 5. Knowledge of computer hardware equipment and software applications relevant to work functions. 6. Ability to communicate effectively both verbally and in writing. 7. Ability to perform crucial conversations with desired outcomes. 8. Ability to establish and maintain effective working relationships with all levels of personnel and medical staff. 9. Ability to problem solve and engage independent critical thinking skills. 10. Ability to maintain confidentiality relevant to sensitive information. 11. Ability to prioritize work demands and work with minimal supervision. 12. Ability to maintain regular and punctual attendance. EDUCATION AND EXPERIENCE: Associate's degree required. Enrollment in National Cancer Registrars Association (NCRA) approved Oncology Data Specialist classes upon hire. Prior experience in clinical abstraction in cancer registry or related field desired. Oncology Data Specialist certification required within three (3) years of date of hire.
