The Opportunity Works closely with individuals and communities to promote and maintain good health. Provides education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community. Conducts community outreach and engage community members to understand their health needs. Provides individual and group health education and coaching to community members. Assists community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services. Provides basic health screenings and assessments to identify health risks. What You'll Bring Education High School Diploma or Equivalent required Associate's Degree Social Work preferred Bachelor's Degree in related field of study preferred Required Experience 1+ years of experience in community outreach, health education or related field Strongly Preferred Experience Experience with community-based work, patient home visits, SUD, homelessness and a complex patient population Experience with Social Determinants of Health (SDOH), housing, food insecurity, transportation, utilities, employment, childcare assistance programs Experience helping patients or clients apply for state/federal programs/benefits (i.e. SSDI, SSI, SNAP) and advocating for the patient with appeals Knowledge of transportation resources (PT1) Knowledge, Skills & Abilities Strong communication and interpersonal skills, with the ability to interact effectively with various populations. Demonstrated ability to work effectively and provide advocacy for all populations and communities preferred. Ability to work independently and as part of a team. Basic computer skills, including Microsoft Office and database management. Bilingual preferred.
