Job Summary: Incumbent will assist students in navigating college, state and community resources to reduce food and housing insecurity. Provide individualized support to students experiencing food and/or housing insecurity, helping them navigate available college, state, and community resources. Minimum Qualifications: Bachelor's Degree and 3 years related experience, or a combination of experience and education sufficient to successfully perform the essential duties of the job. Job Specific Task List:
- Provide individualized triage/case management to a diverse student population across a broad range of presenting basic need issues; serve as single point of contact at assigned college campuses for resources on homelessness, food insecurity, and other socio-economic barriers experienced by students. - (Essential)
- Implement policies and procedures in collaboration with the AVP of Holistic Support and Engagement and various College departments to best meet the basic needs of individual students and the broader student population. - (Essential)
- Meet with students (appointment & walk-in basis) to assess their immediate or emergency needs, and provide referral and intervention services as needed; manage participant intake; provide referrals to appropriate community services and social service agencies for additional services as required; responsible for ensuring timely, effective, and professional responses and referrals; maintain consistent follow up on referrals. - (Essential)
- Serve as resource person to college staff and faculty regarding student wellbeing issues, community support services, and programs; develop appropriate databases, resource guides, and information packets for faculty, students, and other college members. - (Essential)
- Create and maintain a professional, welcoming environment for students visiting the HACC CARE Center for services and information; maintain accurate confidential records (including electronic records) of interactions with students; track service delivery and outcomes. - (Essential)
- Conduct student interactions in compliance with FERPA regulations; demonstrate understanding of the College's interpretation of FERPA rules for the release of student information to faculty, parents, students, and third parties. - (Essential)
- Develop and maintain relationships with campus and community partners in HACC service area to identify resources for HACC students; research external opportunities for support emergency support services and basic needs resources. - (Essential)
- Work with internal and external partners to coordinate workshops and professional development for students, faculty, and staff on critical topics pertaining to financial literacy, food insecurity, homelessness, accessing safety net programs, understanding health insurance, free/low cost legal clinics, tax prep services, and other similar programs and services. - (Essential)
- Plan, implement, evaluate, and market trainings, programs, and informational sessions in collaboration with campus and external community partners; organize campus and campus-community events to bring awareness and improve access to information and resources; represent the College in external committees on issues associated with student basic needs. - (Essential)
- Provide on-going support for data collection, program assessment and reporting; regularly collect, monitor, analyze, and report on operational and outcome data from services. - (Essential)
- Travel to HACC locations to support HACC students; attend training/professional development opportunities related to responsibilities. - (Essential)
- Conduct needs assessments to identify barriers to student success and create personalized resource connection plans. (Essential)
- Collaborate closely with the CARE Center, Student Access Services, Financial Aid, and other campus and community partners to coordinate holistic support. (Essential)

- Responsible for scheduling student/staff/community meetings, reconciling purchasing card transactions, maintaining office supplies, triaging student/staff phone calls, preparatory tasks for events, and other duties as assigned. - (Essential)
