Job Description:

In this role, you will evaluate and treat patients with speech, language, communication, cognitive, hearing, and swallowing (dysphagia) disorders, delivering personalized therapy in the comfort of their homes. As a valued member of our interdisciplinary team, you will help patients regain independence and improve their quality of life through individualized, patient-centered interventions.

The Speech Language Pathologist (SLP) is responsible for overseeing and ensuring a comprehensive rehabilitation program designed to meet the communicative, cognitive and/or swallowing needs of the patient. This includes promoting client/caregiver ability for self-determination and care, optimizing their participation in this process and optimizing outside resource utilization to achieve self-management. You will also coordinate and communicate with various disciplines or services involved in client care and coordinating their efforts toward discharge goals established at onset and during course of care. You will also be a patient advocate.

Additional Job Duties:

Evaluates and treats patients regarding the application of a wide variety of therapeutic interventions for rehabilitation of speech, language, communication, cognition, hearing and dysphasia disorders.

Must demonstrate knowledge of the principles of growth and development over the life span of patients served

Oversees and insures a comprehensive rehabilitation program designed to meet the physical, psychosocial, cognitive and immediate environmental well-being of the patient.

Promotes patient/caregiver self-determination by optimizing outside resource utilization to achieve the highest level of independence.

Coordinates and communicates with various disciplines or services involved in patient care.

Coordinates efforts toward discharge goals established at initial evaluation and during episode of care.

Is a patient advocate.

Other duties as assigned.

Settings may include inpatient, outpatient, long term care, community based care and home health care

Requires detailed knowledge of speech and language pathology in terms of what and how work is to be done as well as why it is done, this level includes interpretation of data

Basic understanding of the rules and regulations of the specific setting

Qualifications:

  • Masters degree in Speech and Language Pathology

  • Certificate of Clinical Competence

  • SLP Licensure in the State of New Mexico

  • Speech Language Pathologist

  • BLS Required at time of Hire

  • Must meet all the educational requirements of Home Health Medicare Conditions of Participation if necessary.
