What You'll Do As a Physical Therapist, you will play a vital role in delivering comprehensive, patient-centered care. Reporting to the Rehabilitation Team Lead, you will: Perform comprehensive evaluations, assessments, and diagnoses Develop and implement individualized care plans tailored to each patient Provide hands-on therapeutic interventions in a home care setting Educate patients and families to support long-term health and recovery Collaborate with a multidisciplinary team, including specialty programs such as: Parkinson's Disease care COVID-19 recovery Post-acute stroke rehabilitation Respond to changing patient needs with clinical judgment and adaptability Why Join Us?
