ABOUT THE JOB The role of the hospice social worker is to deliver varied social work services to Hospice patients and their families. This may include emotional, spiritual, and psychosocial assessments, as well as ongoing counseling, bereavement services and community education, outreach, and referral. The hospice social worker is an integral part of the hospice interdisciplinary team. This position will work approximately 24 hours per week. Responsibilities:
- Functions as a member of the Hospice Interdisciplinary Team
Performs initial psychosocial assessment on every hospice patient to identify needs, determine the medium of service delivery, and complete follow through to ensure provision for social, emotional, and psychological needs of the patient and family. Provides psychosocial input into the development of the patient plan of care. Assist the team in understanding the significance of psychosocial factors on the overall health and well-being of the family unit facing serious illness. Assists with the emergency or direct placements to SNF/NF/AFL facilities as needed. Provides orientation and ongoing education to staff and volunteers on social work principles. - Completes necessary forms and documentation regarding assessment and observations for inclusion into patient's medical record.
- Assists with Advanced Directives and POLST education with staff and patients as needed and as available.
Assumes primary coordination and facilitation responsibility for grief support program and bereavement portion of hospice services:
Promotes bereavement services. Is available for individual grief counseling. bereavement volunteers. Organizes annual memorial service for families and bereaved.
Assists family and/or significant others during time of admission, crisis, or discharge helping them to understand the changes that may occur within the family unit. - Acts as a liaison and support person on behalf of the patient and/or family amid the hospice team and community resources and accesses those resources when necessary
Assists in implementing new policies and procedures regarding social services. Other duties as assigned.
ABOUT YOU
- Master's degree in social work
- 1+ year of experience
- Current LGSW, LISW, or LICSW in the State of Minnesota
Excellent organizational and problem-solving skills Ability to read and interpret documentation Basic computer skills including Excel, Outlook, Word, etc. Ability to manage stress & emotional challenges Ability to act and make decisions independently based on the values of Olivia Hospital & Clinic Strong critical thinking skills Effective communication skills
