Position:RN Case Manager, Home Health Position Type:Full-Time Coverage Area: Madison County and Surrounding Areas, MS Pay: $71,000-$79,000 per year This position is paid on a per-point basis. As a Home Health Registered Nurse (RN), you will lead and manage patient care from admission through discharge-bringing clinical expertise, compassion, and coordination to every interaction. You'll serve as the central point of contact for patients, families, physicians, and the interdisciplinary care team, ensuring care plans are executed safely, effectively, and with a focus on quality outcomes. In this role, you'll combine hands-on nursing care with care coordination, education, and clinical oversight to support each patient's unique journey.
