Mass General Brigham Population Health Services Organization (PHSO) is seeking an experienced Advanced Practice Provider (APP) to join an innovative, multidisciplinary team focused on improving outcomes for Medicare beneficiaries undergoing major surgical procedures. This role supports the Centers for Medicare & Medicaid Services (CMS) Transforming Episode Accountability Model (TEAM), an episode-based care model designed to enhance quality, coordination, and patient experience while reducing unnecessary utilization and readmissions.
