The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Job Overview: The Advanced Practice Nurse (APN) in the outpatient heart transplant setting provides advanced, evidence-based care across the transplant continuum, including pre-transplant evaluation and listing, and longitudinal post-transplant management of immunosuppression, rejection surveillance, and transplant-related comorbidities Practicing within an established collaborative practice agreement, the APN performs comprehensive assessments, formulates and implements management plans, orders and interprets diagnostic studies, prescribes and adjusts medications, and coordinates care alongside transplant cardiologists, surgeons, transplant coordinators, transplant pharmacists, social workers, dietitians, and other interdisciplinary team members Requirements:
