MSW Inpatient Case Manager- Contingent

What to expect in this role:

You'll provide therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated and uninterrupted care in the hospital and to facilitate continuity of care in the community. You will conduct patient assessments, develop care plans, coordinate with providers and serve as a patient advocate.

* After 6 months, you'd have the option to work weekends and holidays remotely with equipment provided.

* The Case Management department has recently redesigned into a dyad Social Work/RN Case Manager model where UR is remote, allowing staff to excel in their chosen profession.
